Join the Effort to Improve Community Health!

We are recruiting community members for a focus group at the West Cook YMCA on Tuesday, October 22 from 6pm - 7:30pm.

Our discussion will be focused on community members’ thoughts about what makes a healthy community and their input about strategies that could be used to improve health in your community and other communities in Chicago and Suburban Cook County.

The Alliance for Health Equity, a collaboration of over 30 hospitals, local health departments, and community organizations in Chicago and Suburban Cook County, is conducting a Community Health Needs Assessment. Your input is crucial in shaping a healthier future for our community!

We need diverse voices to ensure everyone's health needs are represented. This information will guide hospitals and health departments on where to allocate community benefit funding, support existing programs, and implement new health improvement strategies.

Plus, all focus group participants will receive a $25 Target gift card!
